OMHA (Sept 3) and OWHA (Sept 27) have implemented COVID vaccination policies for all eligible participants to be fully vaccinated including a 14 day wait period -- by October 31.
Lucan Minor Hockey is required to validate all individuals in order for participation to continue.
The individual will no longer be eligible to participate in any sanctioned activities until such time as they have presented satisfactory evidence that they have been fully vaccinated or been granted an accommodation.
These requirements are not associated with and do not impact/change any of the requirements to enter an ice facility (Lucan or other centres).
The scope of the policy includes all players, team staff, officials, game staff (e.g. timekeepers/gate), and volunteers (e.g. Exec, events). Only players and spectators born in 2010 or after are exempt from this policy.
Please complete the following form on or before October 1 with your current status (vaccinated, partially, etc). You will complete the form a second time if you are partially vaccinated and then become fully vaccinated.
