LMH is intending completed coaching assessments to be a communication tool for both recognition of positive attributes as well as constructive-feedback intended to provide assistance to our coaches and the overall LMH program.
Please be frank, honest and constructive in your responses. If you are completing the survey due to concerns that you have, we ask that you give 24 hours prior to submitting your survey to ensure that you take some time to think through your feedback and the issue at hand.
We recognize that coaching assessments may contain sensitive information and we will respect that you are submitting the information under the assumption that it will be confidential. In order to use the information provided in a constructive fashion please be aware any feedback provided may be shared (in confidence) with the coaches in question, the coaching committee, and potentially the LMH Executive as deemed appropriate.
